When to Use
- Use R764 for the surgical exploration of a major artery when no other definitive vascular procedure (such as a bypass or repair) is performed.
- Use this code when the clinical intent is diagnostic or exploratory in nature, such as identifying the source of an arterial obstruction or assessing vessel integrity in a trauma setting.
Common Pitfalls
- Do not bill Z402 (Arteriotomy) in addition to R764, as Z402 is explicitly restricted when performing major cardiovascular surgery like R764.
- Avoid billing R764 as a primary procedure if a more definitive vascular procedure (e.g., R775, R776, or R781) is performed during the same operative session, as the exploration is considered integral to the definitive repair.
Billing Tips
- If performing an embolectomy or thrombectomy during the exploration, ensure you append the E649 add-on code to the claim to capture the additional work performed.
- Ensure the trauma premium (E420) is only applied if the patient meets the specific Injury Severity Score (ISS) thresholds and the score is documented in the medical record.
