Resection or repair of an AV fistula aneurysm(s), includes any necessary repair, with or without thrombectomy. As per , only one of R941, R942, R943, R944, R945 or R946 is eligible for payment per patient per day, any physician.

When to Use

  • Use R945 when performing a formal resection of a symptomatic or enlarging AV fistula aneurysm, regardless of whether a thrombectomy is required during the same session.
  • Select R945 for repairs involving significant reconstruction of the fistula site that exceeds the scope of simple revision or ligation codes.

Common Pitfalls

  • Billing R945 in combination with R941, R942, R943, R944, or R946 on the same day will trigger an automatic rejection due to the global restriction per patient per day.
  • Attempting to unbundle thrombectomy as a separate procedure is a common audit risk; R945 explicitly includes any necessary thrombectomy in its fee.
