S157 – Colostomy

Colostomy is a surgical procedure, classified as an enterotomy, to create an opening for the large intestine. The service includes the pre-operative evaluation, the anaesthetic procedure, and post-anaesthetic follow-up. Payment is calculated on a unit basis for anaesthesia and assistant services. When to Use
- Use S157 for the creation of a permanent or temporary colostomy when performed as a standalone procedure or as the primary surgical intent.
- Select S157 when performing an enterostomy specifically on the large intestine, distinguishing it from small bowel procedures like an ileostomy (S149).
Common Pitfalls
- Failing to append the E793 premium when the colostomy is performed laparoscopically, which results in a 25% loss of the potential surgical fee.
- Attempting to bill for a second surgical assistant without prior authorization, as S157 is not on the pre-approved list under GP90:104.
- Confusing S157 with S154 or S156; ensure the procedure is strictly a colostomy creation and not a colostomy closure or revision.
