S165 – Resection with anastomosis - small intestine - other

A surgical procedure involving the resection of a segment of the small intestine, other than the duodenum, and the subsequent re-connection (anastomosis) of the cut ends. This procedure is eligible for assistant (<suffix>B</suffix>) and anaesthetist (<suffix>C</suffix>) fees which are calculated based on basic and time units specified in the Schedule of Benefits. See S165B and S165C in the add-on codes section for details. This service is also eligible for an add-on fee (E793) if performed laparoscopically.
When to Use
- Use S165 for the resection of a small intestine segment (excluding the duodenum) with primary anastomosis when no other more specific code like S166 (ileostomy) or S167 (colostomy) applies.
- Use S165 for elective or non-elective small bowel resections where the primary intent is the removal of a diseased segment and restoration of intestinal continuity.
Common Pitfalls
- Billing S165 in conjunction with S181, S182, S185, S191, S192, or S193, which is explicitly prohibited when claiming the E718 add-on.
- Failing to append the E793 add-on when the procedure is performed laparoscopically, resulting in a missed 25% fee increase.
- Incorrectly coding S165 for duodenal resections; these must be billed under the appropriate duodenal-specific surgical codes.
Billing Tips
- Always verify if the patient has a history of previous resection with anastomosis to determine if the E718 add-on is applicable for an additional $142.40.
- Ensure the E793 add-on is submitted on the same claim as S165 to receive the 25% laparoscopic premium.
