S281 – Choledochoduodenostomy or choledochoenterostomy or choledochocholechostomy

A surgical procedure involving the anastomosis of the common bile duct to another organ to facilitate bile drainage. Specific variations include: - Choledochoduodenostomy: Anastomosis to the duodenum. - Choledochoenterostomy: Anastomosis to the jejunum or another part of the small intestine. - Choledochocholechostomy: Anastomosis to the gallbladder. This procedure may be performed for reconstruction after resection of a common duct stricture (see S292). It is not payable with S276.
When to Use
- Use S281 for the definitive surgical creation of a biliary-enteric anastomosis, such as a choledochoduodenostomy, to bypass a distal biliary obstruction.
- Use S281 when performing a choledochoenterostomy as a reconstruction procedure following the resection of a benign or malignant common bile duct stricture.
Common Pitfalls
- Do not attempt to bill S281 in conjunction with S276, as the Schedule of Benefits explicitly prohibits claiming these two codes together.
- Failure to document the specific type of anastomosis (e.g., choledochoduodenostomy vs. choledochoenterostomy) can lead to claim rejections or audit scrutiny regarding the procedure's complexity.
Billing Tips
- If you perform an additional entero-enterostomy during the same operative session, ensure you append add-on code E743 to capture the additional $174.25 fee.
- If a choledochoscopy is performed during the procedure, remember to bill E704 in combination with E721 to receive the $46.50 add-on payment.
