S799 – Bilateral adrenalectomy, with or without oophorectomy

A surgical procedure for the exploration and/or removal of both adrenal glands (bilateral). This procedure may also be performed with or without the removal of one or both ovaries (oophorectomy), as clinically indicated. This code is billable by the surgeon (suffix A). See 'Billing Interactions' for details on billing for surgical assistant (suffix B) and anaesthesia (suffix C) services.
When to Use
- Use S799 for the primary surgical removal of both adrenal glands, regardless of whether an oophorectomy is performed concurrently.
- Use S799 for planned bilateral adrenalectomy in cases of bilateral adrenal hyperplasia or metastatic disease where both glands require resection.
Common Pitfalls
- Do not bill S799 if the adrenalectomy is incidental to a nephrectomy; the adrenal removal is considered part of the nephrectomy procedure.
- Avoid billing S799 for unilateral adrenalectomy; use the appropriate unilateral code (S798) instead to avoid rejection for incorrect procedure scope.
Billing Tips
- Always append the E793 premium to the S799 claim if the procedure is performed laparoscopically to capture the 25% fee increase.
- Ensure the surgical assistant (suffix B) and anaesthesiologist (suffix C) claims are submitted with the correct time units, as these are calculated based on the duration of the S799 procedure.
