X237Bone Mineral Density (BMD) measurement - one site - additional test for other risk factors

OHIP Laboratory Code · Schedule of Benefits

Bone Mineral Density (BMD) measurement by Dual-energy X-ray Absorptiometry (DXA) - by axial technique only. This specific code is for an additional test rendered for patients with specific high-risk factors (Hypercortisolism/Cushing’s syndrome or high-dose glucocorticoid therapy) that justify more frequent testing than standard intervals.

When to Use

  • Use X237 when a patient on high-dose glucocorticoids (>20mg Prednisone equivalent/day) requires annual monitoring and a physical deformity or prosthesis prevents the standard dual-site scan.
  • Use X237 for patients with confirmed Cushing’s syndrome who require more frequent than 36-month monitoring and cannot undergo a two-site scan due to technical limitations.

Common Pitfalls

  • Billing X237 for patients without documented high-dose glucocorticoid use or Cushing’s syndrome will result in audit recovery.
  • Claiming X237 when a two-site scan is technically possible is a violation of the code definition; X236 must be used if both sites are measurable.
  • Submitting X237 within 12 months of any other BMD code (X145, X146, X142, X148, X149, X155, or X236) will trigger an automatic rejection.

Billing Tips

  • Always append the appropriate suffix (B for technical, C for professional) to ensure the claim is processed correctly for the facility and physician components.
  • Ensure the clinical record explicitly justifies why only one site was scanned, as this is the primary requirement for choosing X237 over X236.

Individuals identified as low risk patients may only require repeat BMD testing at 10-year intervals.

BMD testing in patients below age 50 is not routine practice unless specific comorbidities or fragility fractures are present.
