X410CT Abdomen with IV contrast

OHIP Laboratory Code — DIAGNOSTIC RADIOLOGY · Schedule of Benefits

This service is a computed tomography (CT) scan of the abdomen, which includes the administration of intravenous contrast to enhance visualization of blood vessels and organs. Commentary: The maintenance of radiation dose should be consistent with the As Low As Responsibly Achievable (ALARA) principle and current standards under the direction of the radiologist Radiation Protection Officer.

When to Use

  • Use X410 when clinical suspicion of vascular pathology, such as mesenteric ischemia or aortic dissection, necessitates the use of intravenous contrast to enhance diagnostic accuracy.
  • Select X410 for oncological staging or follow-up where contrast enhancement is required to differentiate between solid organ lesions and surrounding vascular structures.
  • Use X410 for the evaluation of inflammatory processes, such as diverticulitis or appendicitis, where contrast is essential to identify abscess formation or bowel wall thickening.

Common Pitfalls

  • Billing X410 alongside X234 is a common cause for automatic rejection, as these codes are mutually exclusive in the OHIP schedule.
  • Failure to document the clinical necessity for contrast enhancement can lead to audit recovery, as non-contrast studies (X409) are the default for routine abdominal imaging.
  • Attempting to claim after-hours premiums (E409, E410) without meeting the 'non-elective' or 'delayed elective' criteria will result in claim denial.

Billing Tips

  • Ensure that the requisition explicitly states the requirement for IV contrast, as this documentation is required to justify the use of X410 over the non-contrast X409 code.
  • When performing urgent after-hours scans in a hospital setting, verify if you qualify for the C104 travel premium in addition to the procedure code, provided the service meets the 'urgent interpretation' requirement.
