Z139 – Aspiration of cyst

This service covers the aspiration of one or more breast cysts. The fee is payable once per session, regardless of the number of cysts aspirated. Assistant services are not payable for this procedure.
When to Use
- Use Z139 for the needle aspiration of a symptomatic breast cyst under clinical or ultrasound guidance.
- Use Z139 when performing a diagnostic aspiration where the fluid is sent for cytology, provided the primary intent is the drainage of the cyst.
Common Pitfalls
- Do not bill Z139 per cyst; the fee is global per session regardless of whether you aspirate one or multiple cysts.
- Avoid billing an office visit (A007) on the same day as Z139, as the assessment is considered included in the surgical procedure fee.
- Do not attempt to bill Z139 in conjunction with a core biopsy (Z141) on the same lesion, as this may trigger a rejection for duplicate or overlapping services.

- Ensure your clinical notes explicitly document the location of the cyst and the volume of fluid aspirated to support the procedure code during potential audits.
- If the procedure is performed after hours in a non-emergency setting, ensure you meet the specific criteria for E409 or E410, as these are the only applicable procedural premiums for this code.
