Needle biopsy of the breast, involving one or more samples. As per the Schedule, this fee code is used whether a single biopsy or multiple are taken in the same session. This service includes all common and specific elements of an insured service as defined in the General Preamble (see pages -).
When to Use
- Use Z141 for percutaneous core needle biopsies of the breast where the intent is tissue sampling for pathology.
- Use Z141 when multiple needle passes are performed on the same breast lesion during a single session, as the code is inclusive of all samples taken.
Common Pitfalls
- Billing Z141 in addition to an office visit code (A007) is generally disallowed as the procedure fee is considered inclusive of the assessment unless the visit is for a separate, unrelated clinical issue.
- Attempting to bill Z141 multiple times for the same session based on the number of cores taken will result in automatic rejection; it is strictly a 'per session' code.
Billing Tips
- Ensure the age premium (15%) is applied if the patient is 65 or older, as Z141 is a surgical procedure code eligible for the age-based premium under the General Preamble.
