  • Use Z341 for the insertion of a chest tube for pneumothorax or pleural effusion when performed as a standalone surgical procedure.
  • Use this code when managing a traumatic hemothorax requiring urgent closed drainage, ensuring it is not bundled with other respiratory surgical procedures performed in the same operative session.

Common Pitfalls

  • Billing a consultation or major assessment on the same day as Z341 will result in a rejection, as Z341 is an Independent Operative Procedure (IOP) that includes the pre-operative assessment.
  • Attempting to bill Z341 alongside Z363 (Thoracoscopy) is prohibited due to explicit code restrictions, which will trigger an automatic claim rejection.
  • Failing to apply the 15% reduction rule when performing a second, lesser procedure during the same hospitalization for a different condition will lead to audit recovery.

Billing Tips

  • Always append applicable Special Visit Premiums (Table I, II, or III) if the procedure is performed in an emergency or hospital setting, as these are permitted add-ons for IOPs.
