Endoscopic Septoplasty. This procedure is designated as an Independent Operative Procedure (IOP) as indicated by the 'Z' prefix. When to Use
- Use Z351 for endoscopic septoplasty when the procedure is performed as an Independent Operative Procedure (IOP) and does not involve concurrent major nasal surgery like functional endoscopic sinus surgery (FESS).
- Use Z351 instead of the traditional septoplasty code Z350 when the surgical approach is specifically documented as endoscopic.
Common Pitfalls
- Billing a consultation or major assessment on the same day as Z351 will trigger a rejection because Z351 is an IOP, which includes the pre-operative assessment in its fee.
- Attempting to bill an assistant fee (M400B) without prior authorization from a medical consultant will result in automatic payment denial.
- Failing to apply the 85% rule when performing Z351 alongside another surgical procedure during the same operative session, as the secondary procedure is subject to the multiple procedure reduction.
Billing Tips
- If the patient is under 16, ensure the appropriate age-based premium is appended to the Z351 claim to capture the percentage increase on the surgeon's fee.
- For non-elective cases performed after hours, append E409A or E410A to the Z351 claim to increase the procedural fee by 50% or 75% respectively.
