Under microscopy, debridement of mastoid cavities, and/or ears with significant external or middle ear pathology but not for removal of cerumen - unilateral.
When to Use
- Use Z907 for the microscopic debridement of a mastoid cavity or ear canal specifically to treat chronic otitis externa or mastoiditis.
- Use Z907 when removing granulation tissue, debris, or crusting from a post-operative mastoidectomy cavity under microscopic visualization.
Common Pitfalls
- Billing Z907 for routine cerumen removal is a common audit trigger; use G420 or an office visit code instead, as Z907 explicitly excludes cerumen management.
- Submitting Z907 without documentation of microscopic visualization will lead to rejections, as debridement performed with loupes or headlights does not qualify.
- Attempting to bill G420 on the same day as Z907 will result in an automatic rejection, as the codes are mutually exclusive.
Billing Tips
- If performing debridement on both ears, bill Z907 for the first ear and Z908 for the second, noting that the second procedure is payable at 85% of the fee.
- Ensure your operative note explicitly states 'microscopic debridement' and describes the specific pathology (e.g., granulation, debris) to justify the use of Z907 over standard office visit codes.

Debridement not performed under microscopy (e.g. if performed using loupes or magnifying headlights) or in the absence of significant external or middle ear pathology, or for removal of cerumen does not constitute Z907 or Z908.
G420 is not eligible for payment in conjunction with Z906, Z907, Z908 or Z913.
For contralateral procedures, Z907 and Z908 are payable at 85% of the listed fee.
