Repair of small perforation under local anaesthesia, with or without debridement, unilateral. As listed in the Schedule, this procedure is distinct from debridement under microscopy (e.g., Z907, Z908) or simple ear syringing (G420).

When to Use

  • Use Z913 when performing a formal repair of a small traumatic or iatrogenic tympanic membrane perforation using local anesthesia.
  • Select Z913 when the procedure involves active debridement of the perforation edges to stimulate healing, distinguishing it from simple diagnostic examination.

Common Pitfalls

  • Billing Z913 alongside G420 will result in an automatic rejection, as the Ministry considers ear syringing part of the preparation for the repair.
  • Attempting to bill Z913 for routine ear wax removal or simple cleaning is an audit risk; ensure the clinical note explicitly describes the perforation repair process.

Billing Tips

  • Always append the appropriate age-based premium if the patient is between one and two years old to increase the base fee by 20%.
