Z914 – Myringotomy with insertion of ventilation tube using operating microscope - unilateral
OHIP Psychiatric Code — AURAL SURGICAL PROCEDURES · Schedule of Benefits
A unilateral procedure for the incision of the eardrum (myringotomy) with insertion of a ventilation tube, performed using an operating microscope. Note: When performed on an otherwise healthy child (defined as age 2 to 15 inclusive per ), Z914 is only eligible for payment for the treatment of symptomatic chronic otitis media with effusion that has persisted beyond 3 months or recurrent episodes of acute otitis media with effusion.
When to Use
- Use Z914 for unilateral myringotomy with tube insertion when the procedure is performed under an operating microscope for chronic otitis media with effusion lasting longer than 3 months.
- Use Z914 for recurrent acute otitis media with effusion in children aged 2 to 15, provided the clinical criteria for frequency and duration are met.
- Use Z914 for unilateral cases; if performing a bilateral procedure, bill Z914 with the appropriate bilateral indicator or as two separate claims depending on current submission requirements.
Common Pitfalls
- Billing Z914 for children aged 2-15 without clear documentation of the 3-month duration or recurrence history, which triggers automatic audit flags.
- Attempting to bill Z914 in conjunction with other ear procedures without ensuring the surgical site and procedure are distinct, as this may lead to rejection for unbundling.
- Failing to use the correct age-based premium code (e.g., AGE_PREMIUM_UNDER_5Y) on the same claim, resulting in underpayment of the surgical fee.
Billing Tips
- Always verify the patient's exact age at the time of service to ensure the correct age-based premium is applied, as these are not automatically calculated by the system.
- Ensure the use of the operating microscope is clearly noted in the operative report, as this is a specific requirement for the Z914 code definition compared to non-microscopic procedures.
