When to Use
- Use E400B for surgical assistance services that commence between 17:00 and 24:00 on weekdays, or anytime on weekends and statutory holidays.
- Apply E400C to anaesthesia services when the case start time falls within the designated evening, weekend, or holiday windows.
- Use E400 in conjunction with C998C or C985C when the anaesthesiologist is called in specifically for an emergency procedure during these time blocks.
Common Pitfalls
- Billing E400 based on the time the procedure ends rather than the actual start time of the case.
- Failing to include the mandatory start and stop times in the medical record when billing E400 in combination with replacement service codes E005B or E005C.
- Confusing E400 (50%) with E401 (75%), which is reserved exclusively for cases commencing between 00:00 and 07:00.

- Ensure the start time is clearly documented in the anaesthesia or surgical record to support the claim if audited against the 17:00 cutoff.
- Always calculate the 50% premium based on the total fee, which must include both the basic units and the total time units for the procedure.
