E400B is a premium that increases the total surgical assistant's fee by 50% when the surgical case commences during specific after-hours periods: Evenings (17:00h – 24:00h) from Monday to Friday, or daytime and evenings on Saturdays, Sundays, or Holidays. The total assistant's fee is calculated by adding basic units and time units, as defined in the Surgical Assistants' Services section of the Schedule (see ). This premium is also payable with the replacement surgical assistant fee (E005B) if the original case commenced during these after-hours periods.
When to Use
- Use E400B when the surgical procedure start time falls within the 17:00h–24:00h window on weekdays, or any time between 07:00h and 24:00h on weekends or statutory holidays.
- Apply this premium to the total surgical assistant fee (basic units plus time units) when acting as a replacement assistant under E005B, provided the original case start time met the after-hours criteria.
Common Pitfalls
- Do not bill E400B for cases starting between 00:00h and 07:00h; these cases require the E401B premium instead.
- Avoid the common error of applying the 50% premium to the Special Visit Premium (e.g., C998B or C983B); E400B only applies to the surgical assistant's procedural fee units, not the flat-fee travel premiums.
- Ensure the start time documented in the operative report matches the start time submitted in the claim, as discrepancies often trigger automated rejections during audits.
Billing Tips
- Always stack E400B with the appropriate Special Visit Premium (C998B, C983B, or C988B) if you traveled from another location to assist, as these are additive and not mutually exclusive.
- Calculate your total units (Basic + Time) first, then apply the 1.5 multiplier to the final dollar amount to ensure the 50% premium is accurately reflected in your total claim value.
