When to Use
- Use E921 when performing a second, distinct intracranial procedure during the same operative session that is not covered by the primary procedure's global fee.
- Apply this code when a repeat craniotomy or cranial exploration is required immediately following the primary procedure, such as for evacuation of a post-operative hematoma before the patient leaves the OR.
Common Pitfalls
- Billing E921 alongside N127 will result in an automatic rejection, as the Schedule of Benefits explicitly excludes this combination.
- Attempting to use E921 for a secondary procedure that is already considered an integral component of the primary intracranial surgery will lead to audit recovery.
Billing Tips
- Ensure E921 is appended to the primary procedure (N111, N114, or N116) to clearly demonstrate the additive nature of the repeat procedure in the same operative encounter.
