E934 – Orbit - open reduction rim/wall fracture - Zygomatic fracture dislocation - with primary bone graft (separate site)
OHIP Surgical Assists Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
This is an add-on code used in conjunction with open reduction of an orbit rim/wall fracture (E173) or a blowout fracture of the orbital floor (E174). It is claimed when a primary bone graft is harvested from a separate anatomical site and used in the orbital reconstruction. This service is an addition to the main procedure.
When to Use
- Use E934 when performing an open reduction of a zygomatic fracture (E173) that requires a primary bone graft harvested from a separate anatomical site, such as the calvarium or iliac crest.
- Use E934 when reconstructing an orbital floor blowout fracture (E174) where the defect size necessitates a primary bone graft harvested from a distinct donor site to ensure structural stability.
Common Pitfalls
- Billing E934 concurrently with Z263, Z264, Z265, Z266, E975, or E976 will result in automatic rejection due to explicit billing restrictions.
- Claiming E934 for the use of synthetic or alloplastic implants is incorrect; this code is strictly for primary bone grafts harvested from a separate anatomical site.
- Failing to link E934 to the primary procedure (E173 or E174) on the same claim will lead to payment errors, as E934 is an add-on code.
Billing Tips
- Ensure the operative report clearly documents the harvesting site of the bone graft to justify the use of E934 over non-grafting procedures.
- Apply relevant age premiums (e.g., for patients under 16) or after-hours premiums (E409/E410) to the total procedural fee, as E934 is considered part of the surgical procedure package.
