This is an add-on fee for the insertion of an intraocular lens. It is claimed in addition to the fee for the primary procedure. It is applicable to: - E141: Dislocated lens extraction - E144: Removal of artificial intraocular lens

When to Use

  • Claim E950 as an add-on when performing a secondary lens implantation during a dislocated lens extraction coded as E141.
  • Use E950 when replacing an existing artificial lens that requires removal via E144 and subsequent insertion of a new lens.

Common Pitfalls

  • Submitting E950 alongside E156 or E157 will result in an automatic rejection due to explicit billing restrictions.
  • Attempting to claim E950 as a standalone procedure will lead to rejection, as it is strictly an add-on code requiring a primary surgical procedure code.
