Team management in a Rehabilitation Unit is for active in-patient rehabilitation management. This fee code applies to visits from the thirteenth to the twenty-sixth week of care. When this service is rendered by one physiatrist, even if part of the service is rendered in an active treatment hospital and part in a rehabilitation unit, the weekly and monthly limitations under fee codes H312, H317, and H319 apply to the total rehabilitation care rendered. It is not possible to claim the maximum fees allowed under C312, C317, and C319 and then start claiming de novo under the H-prefix codes under these circumstances.

When to Use

  • Use H317 for active in-patient rehabilitation management specifically between the 13th and 26th week of a patient's continuous rehabilitation stay.
  • Apply this code when managing a patient in a designated rehabilitation unit who has already exhausted the initial 12-week period covered by H312.

  • Attempting to claim H317 immediately after reaching the maximums for C317 in an active treatment hospital, which violates the continuous care rule for rehabilitation.
