A Level 1 overnight sleep study with continuous monitoring of oxygen saturation, ECG and Ventilation (airflow and respiratory effort) and additional monitoring to stage sleep (including all of the following: EEG, EOG and sub-mental EMG). This service is an "initial diagnostic study”, defined as the first overnight sleep study rendered to an insured person as an insured service in Ontario for the purpose of establishing the diagnosis of a sleep disorder (and includes a split night study). Every overnight diagnostic sleep study rendered before July 1, 2010 for which a claim was submitted and paid as an insured service under the Health Insurance Act constitutes an “initial diagnostic study” and is deemed to have been rendered on July 1, 2010.
When to Use
- Use J896 for the very first overnight polysomnography performed on a patient to diagnose a sleep disorder, including split-night studies where titration occurs in the second half.
- Select J896 when the study includes the full required montage of EEG, EOG, and sub-mental EMG to properly stage sleep, distinguishing it from limited channel home sleep apnea testing.
Common Pitfalls
- Billing J896 for a patient who has previously had an insured sleep study in Ontario will result in a rejection, as this code is strictly reserved for the 'initial diagnostic study'.
- Submitting J896 when the recording quality is insufficient for interpretation is a major audit risk; in such cases, you must bill J898, J899, or J990 based on the time in bed instead.
Billing Tips
- Ensure the referral from a physician or nurse practitioner is documented and on file, as J896 strictly requires a formal referral to be considered an insured service.
