Replacement of aortic valve, replacement of ascending aorta, and reimplantation of coronary arteries (Modified Bentall). A second assistant's services are payable without authorization (). Multivalvular replacement: the fee is that for the major valve replaced plus 85% of the fee for the additional valve or valves. E516 (endocarditis complexity premium, add 25%) applies when the procedure is complicated by active infective endocarditis.
