S647 – Prostatectomy, suprapubic - with or without removal of bladder stones

A surgical procedure for the excision of the prostate gland, performed via a suprapubic approach. The service fee is inclusive of a vasectomy when one is performed at the same time. It does not, however, include an investigative cystoscopy. An additional premium, E792, is payable if this procedure is performed laparoscopically.
When to Use
- Use S647 for open suprapubic prostatectomy where the bladder is accessed directly to remove the gland.
- Use this code when a vasectomy is performed concurrently, as it is considered inclusive and should not be billed separately.
Common Pitfalls
- Do not bill an investigative cystoscopy separately, as it is explicitly excluded from the S647 fee.
- Failure to append E792 when the procedure is performed laparoscopically will result in a 25% underpayment of the surgical fee.
- Billing a separate vasectomy code is a common error; the S647 fee already covers this service.
