Radical retropubic prostatectomy, performed with or without the removal of bladder stones. Per note on , payment for prostatectomy codes S650 and S651 does not include investigative cystoscopy but does include vasectomy when rendered. Payment for S651 also includes S519 (plastic repair of bladder neck) and/or S636 (vesiculectomy) when rendered during the same operative session.
When to Use
- Use S651 for a radical retropubic prostatectomy where a concurrent vesiculectomy (S636) or plastic repair of the bladder neck (S519) is performed.
- Use S651 when a vasectomy is performed as part of the radical prostatectomy procedure, as it is considered included in the fee.
Common Pitfalls
- Billing S519 or S636 as separate line items is a common error that will result in a rejection, as these are explicitly included in the S651 fee.
- Failing to bill an investigative cystoscopy separately when performed; unlike the prostatectomy itself, the cystoscopy is not included and should be claimed using the appropriate Z-code.
Billing Tips
- Always apply the 15% age premium if the patient is 65 years or older, as this is a surgical procedure eligible for the age-based increase.
