S646 – Prostatectomy - perineal with vesiculectomy

Perineal prostatectomy with vesiculectomy. This is a surgical procedure involving the excision of the prostate gland along with the seminal vesicles, performed via an incision in the perineum. As per Note 1 on page of the Schedule of Benefits, this procedure includes vasectomy when rendered but does not include investigative cystoscopy.
When to Use
- Use S646 when performing a perineal prostatectomy that specifically includes the removal of the seminal vesicles (vesiculectomy).
- Use this code when a vasectomy is performed concurrently with the perineal prostatectomy, as the vasectomy is considered included in the S646 fee.
Common Pitfalls
- Do not bill investigative cystoscopy separately, as it is explicitly excluded from the S646 fee and will be rejected if billed in addition to the prostatectomy.
- Avoid billing a separate fee for a vasectomy when performed during this procedure, as it is already bundled into the S646 payment.
Billing Tips
- Ensure that if an investigative cystoscopy is medically necessary, it is documented as a separate diagnostic event, but be aware it remains non-billable when performed during the same operative session as S646.
