A colonoscopy for diagnosis or ongoing management, indicated by the presence of signs or symptoms. The procedure involves an examination of the large intestine from the sigmoid colon to the descending colon.
When to Use
- Use Z496 when performing a diagnostic colonoscopy specifically limited to the sigmoid and descending colon due to documented clinical signs or symptoms.
- Select Z496 for follow-up management of known pathology when the scope is restricted to the distal colon, provided the patient presents with active symptoms.
Common Pitfalls
- Billing Z496 in combination with any other code from the Z491-Z499 or Z555 series on the same day will result in a rejection, as only one is permitted per patient per day.
- Attempting to bill E003C for anaesthesia services alongside Z496 is explicitly prohibited and will lead to claim adjustment.
Billing Tips
- Ensure the procedural note clearly justifies the clinical indication for the procedure, as Z496 is strictly for patients with signs or symptoms rather than routine screening.
- Verify the patient's age at the time of service to automatically apply the correct percentage-based age premium, as these are significant additions to the base fee.
