- Choose Z582 when the procedure is limited to visual inspection and does not involve operative interventions like biopsy or polypectomy, which would require different codes.
Common Pitfalls
- Billing Z582 in conjunction with S772 (endometrial ablation) will result in a rejection, as the diagnostic procedure is considered bundled into the ablation.
- Submitting Z582 alongside Z583, Z585, Z586, or Z587 on the same day will trigger a rejection because only one hysteroscopy code is payable per patient per day.
Billing Tips
- If you require a surgical assistant, you must obtain prior authorization from a medical consultant and bill using M400B with 6 basic units.

Only one of Z582, Z583, Z585, Z587 or Z586 is eligible for payment for the same patient on the same day.
Hysteroscopy (Z582, Z583, Z587) is not eligible for payment when rendered with endometrial ablation (S772).
Assistant services (suffix B) are only eligible for payment upon authorization by a medical consultant. Claims should be submitted using fee code M400B with basic units equal to the anaesthesia units (6) for this service.
