Hysteroscopy with uterine biopsy and/or D&C is an endoscopic surgical procedure performed on the corpus uteri. It involves visualizing the uterine cavity and may include taking a tissue sample (biopsy) or scraping the uterine lining (dilation and curettage).
When to Use
- Use Z583 when performing a diagnostic hysteroscopy combined with a targeted endometrial biopsy or a therapeutic D&C for abnormal uterine bleeding.
- Select Z583 when the primary intent is to visualize the uterine cavity and obtain tissue, distinguishing it from Z582 which is strictly for diagnostic hysteroscopy without biopsy or curettage.
Common Pitfalls
- Do not bill Z583 in conjunction with S772 (endometrial ablation), as the Ministry explicitly prohibits payment for both on the same day.
- Avoid billing Z583 alongside Z582, Z585, Z586, or Z587; the system will reject the claim as only one hysteroscopic procedure code is permitted per patient per day.
- Ensure the procedure is not billed as a standalone office visit; Z583 is a surgical procedure code and should not be submitted with a general assessment code unless a separate, identifiable service is provided.
Billing Tips
- If the patient is between 5 and 16 years of age, remember to apply the 10% age-based surgical premium to the Z583 fee.
- For non-elective procedures performed after hours, ensure the appropriate E409 or E410 premium is appended to maximize the procedural reimbursement.
