When to Use
- Use Z585 for hysteroscopic lysis of intrauterine adhesions (synechiae) when the total procedure time is less than 60 minutes.
- Use Z585 when performing hysteroscopic cannulization of the fallopian tubes, regardless of whether lysis of adhesions is also performed.
Common Pitfalls
- Billing Z585 and Z586 on the same day is a hard rejection; you must choose the code that reflects the total duration of the lysis procedure.
- Attempting to bill Z585 alongside Z582, Z583, or Z587 will result in a payment denial as these are mutually exclusive on the same date of service.
- Failure to document the start and end time of the lysis procedure can lead to audit recovery if the claim is challenged for being Z586 (60+ minutes).
Billing Tips
- Always document the exact duration of the lysis of adhesions in the operative report to justify the use of Z585 over Z586.
