Z720 – Biopsy - with or without fulguration
OHIP Psychiatric Code — FEMALE GENITAL SURGICAL PROCEDURES · Schedule of Benefits
Z720 is a surgical procedure for a biopsy of the cervix, with or without fulguration. As per a note on page of the Schedule, this code should also be used for the excision of cervical polyp(s) when performed under general anaesthesia. The service includes 6 anaesthesia base units.
When to Use
- Use Z720 for a diagnostic biopsy of the cervix performed in an office or surgical setting.
- Use Z720 for the excision of cervical polyp(s) specifically when the procedure is performed under general anaesthesia.
- Use Z720 for cervical biopsy with fulguration of the base, provided no colposcopy codes are claimed for the same encounter.
Common Pitfalls
- Claims will be rejected if submitted with colposcopy codes Z731, Z787, or Z730, as these are mutually exclusive.
- Do not bill Z720 for cervical polyp excision if performed under local anaesthesia; this is typically considered part of the office visit or a different procedure code.
- Attempting to bill Z720 in addition to a consultation or assessment code on the same day often leads to rejection unless a valid diagnostic code and appropriate modifier are used to justify the separate service.
Billing Tips
- Ensure the claim includes the 6 anaesthesia base units when the procedure is performed under general anaesthesia to correctly trigger the anaesthesiologist's fee.
- If performing a cervical polyp excision under general anaesthesia, explicitly reference the note on page 734 of the Schedule in your clinical records to support the use of Z720 over other potential surgical codes.
