A962 – Travel Premium

A travel premium payable to a physician who travels from one location to their office (the destination) to provide a non-elective insured service to a patient during evening hours (17:00 to 24:00) Monday through Friday, provided the office is not otherwise scheduled to be open.
When to Use
- Bill A962 when you travel from home to your office after 5 PM on a weekday to see a patient, and your office is not otherwise scheduled to be open.
- Use A962 if you leave a hospital or another patient's home after 5 PM on a weekday and travel directly to your office to see a patient, provided the office is closed.
- This code is appropriate when you travel to your office after 5 PM on a weekday specifically to see a patient, and you are not already at the office for other scheduled duties.
Common Pitfalls
- Claims for A962 are rejected if the physician is already at the office for other scheduled activities, even if travelling from another location.
- Do not bill A962 if the office is open for scheduled appointments during the time of your arrival and patient visit; this premium is only for travel to a closed office.
- A962 cannot be billed with 'H' prefix codes, as these represent emergency department services which have different premium structures.
Billing Tips
- Ensure your medical record clearly documents the exact time of travel and the patient encounter to support the A962 claim.
- A962 is a premium for the travel itself; it must be billed in conjunction with an eligible A-prefix assessment code for the patient visit.

The time at which the special visit takes place must be documented on the medical record.
The physician must personally travel to the destination.
Only one travel premium is eligible for payment for each separate trip to a destination regardless of the number of patients seen.
Additional person seen premiums are not eligible in the Physician Office setting (Table VIII).
Not eligible for virtual care services (not listed in Appendix J).
