A travel premium for a non-elective special visit to the physician's office, for visits that commence Monday to Friday between 17:00 and 24:00. Fee $37.40 (max. 1 per time period). Schedule of Benefits Special Visit Premium Table VIII, Physician Office (). The time the special visit takes place must be documented in the medical record (). Bill with the appropriate A-prefix assessment from the General Listings.

The physician must personally travel to the destination.
Usage limits
Maximum 1 per time period (Evenings 17:00-24:00 Monday through Friday).
Only one travel premium is eligible for payment for each separate trip to a destination regardless of the number of patients seen.
Additional person seen premiums are not eligible in the Physician Office setting (Table VIII).
Not eligible for virtual care services (not listed in Appendix J).
