A travel premium for a non-elective special visit to a patient's home (other than a long-term care institution), for visits that commence on a weekday between 07:00 and 17:00 with sacrifice of office hours. Fee $37.40 (max. 2 per time period). Schedule of Benefits Special Visit Premium Table VI, Patient's Home (not long-term care) (). The time the special visit takes place must be documented in the medical record (). B960 and B990 maximums per physician per day count non-elective and elective visits together. Not payable with A007 or A001 rendered in the home. Bill with the appropriate A-prefix assessment from the General Listings.
Only one travel premium is eligible for payment for each separate trip to a destination regardless of the number of patients seen in association with each trip.
Maximum 2 services per time period.
