A travel premium payable for a non-elective special visit to a patient's home (other than a long-term care institution) commenced during the night (00:00 to 07:00).
When to Use
- Bill B964 when called to a patient's private residence between midnight and 7 AM for an urgent, non-elective reason, such as a sudden severe illness or injury.
- Use B964 for a patient requiring an unscheduled home visit overnight due to a worsening chronic condition that cannot wait until morning, distinct from routine follow-ups.
Common Pitfalls
- Do not bill B964 if the visit occurs at a retirement home or group home; these are considered institutions and do not qualify for this premium.
- Avoid billing B964 if the visit is for an elective service or a routine follow-up, even if it occurs during the night; these situations require different codes like B960.
- Ensure the visit is a 'special visit' initiated by the patient or on their behalf for a non-elective reason; routine scheduled visits do not qualify.
Billing Tips
- B964 can be billed with B996 (First person seen - Nights) when you are the first physician to see a patient during a night visit to their home.
- This premium is not restricted by the number of times it can be billed per period, unlike daytime or evening travel premiums.

The visit must be a 'special visit' (initiated by the patient or an individual on behalf of the patient for a non-elective service).
The service must be commenced between 00:00 and 07:00 (Monday through Friday).
Travel must be from one location to another location (the destination).
A multiple resident dwelling (e.g., apartment, retirement home, group home) is considered a single destination; only one travel premium is payable for the first patient seen at that location.
Travel premiums are only eligible when rendered with certain services listed under 'Consultations and Visits' and 'Diagnostic and Therapeutic Procedures'.
