It applies in two contexts as per the Schedule of Benefits: 1. **Cervical Spine (below ):** Performed in addition to decompression procedures N509, N510, N572, N574, N575, N576, N560, or N561. 2. Thoracic & Lumbar Spine: Performed in addition to decompression or osteotomy procedures N509, N510, N511, N512, N572, N574, N575, N576, N560, or N561. Additional fused levels are claimed using other codes such as E366.

When to Use

  • Use E370 when performing a posterior spinal arthrodesis with instrumentation at a single level following a cervical decompression procedure such as N509 or N510.
  • Use E370 for a single-level thoracic or lumbar fusion performed in conjunction with a decompression procedure like N511 or N512.
  • Use E370 as the primary code for the first level of fusion when multiple levels are addressed, provided the additional levels are claimed using E366.
