It applies in two contexts as per the Schedule of Benefits: 1. **Cervical Spine (below ):** Performed in addition to decompression procedures N509, N510, N572, N574, N575, N576, N560, or N561. 2. Thoracic & Lumbar Spine: Performed in addition to decompression or osteotomy procedures N509, N510, N511, N512, N572, N574, N575, N576, N560, or N561. Additional fused levels are claimed using other codes such as `E366.
All insured services must be documented in appropriate records establishing that the service was provided, is the service for which the account is submitted, and was medically necessary.
