When to Use
- Use E688 when you perform active laser debulking of a rectal or sigmoid tumor or polyp in addition to the primary excision or fulguration code (e.g., Z753, Z754).
- Apply this code specifically when the laser is used to reduce the bulk of a lesion that cannot be fully removed by snare or forceps alone.
Common Pitfalls
- Billing E688 without an eligible primary procedure code (Z753, Z754, Z784, Z785, Z755, or Z761) will result in an automatic rejection.
- Exceeding the maximum of 2 units per patient encounter, as E688 is strictly limited to two applications regardless of the number of lesions treated.
- Attempting to bill E688 for routine cautery or standard snare polypectomy where no laser debulking was actually performed.

- Ensure your operative report explicitly mentions the use of laser technology for debulking to justify the additional fee over the base procedure code.
