Excision of polyps or tumours of the rectum or sigmoid with a base under 2 cm. Notes: - To a maximum of 2, any size or technique, from the group of procedures Z753 to Z761. - For fulguration or excision of tumours through the colonoscope, refer to page . - The surgical assistant fee (Z754B) is based on 6 base units plus time units as per -. - The anaesthesia fee (Z754C) is based on 6 base units plus time units as per -. - This service is denoted by the '#' prefix, indicating that premises, equipment, supplies, and personnel are funded by the hospital global budget or ICHSC facility costs and cannot be charged to the patient ().
When to Use
- Use Z754 for the surgical excision of a rectal or sigmoid polyp with a base diameter of less than 2 cm when performed via proctoscopy or sigmoidoscopy.
- Use this code when performing a second excision of a polyp or tumour in the same session, as the Schedule allows for a maximum of two procedures from the Z753-Z761 group.
Common Pitfalls
- Do not bill Z754 for polyps removed via colonoscopy, as these must be billed under the colonoscopy section (S19) rather than the surgical excision section.
- Billing more than two procedures from the Z753-Z761 group in a single session will trigger an automatic rejection; ensure you are not exceeding the maximum of two.
- Attempting to bill a hospital admission assessment alongside Z754 is disallowed unless it qualifies as the major pre-operative visit where the decision to operate was finalized.
Billing Tips
- Always apply the 15% age-based premium (AGE_PREMIUM_65_PLUS) if the patient is 65 years of age or older to maximize the claim value.
- If the procedure is performed after hours, ensure you append the appropriate E-code (E409, E410, E400C, or E401C) based on the specific time window to receive the correct premium.
