Z761 – Electrocoagulation of rectal or sigmoid polyp or tumour - base over 5 cm
OHIP Psychiatric Code — DIGESTIVE SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
This service covers the electrocoagulation of large polyps or tumours (base over 5 cm) located in the rectum or sigmoid colon. It is a surgical procedure that allows for both an assistant and an anaesthetist. A note on page specifies a maximum of 2 procedures from the 'Polyps or tumours of rectum or sigmoid' group (codes Z753 to Z761) are payable per day, regardless of size or technique. For fulguration or excision of tumours via colonoscope, refer to services on page . As a '#' prefixed code, facility costs are included when performed in a hospital ().
When to Use
- Use Z761 when performing electrocoagulation of a rectal or sigmoid polyp or tumour with a base exceeding 5 cm via open or endoscopic approach, excluding colonoscopic procedures.
- Use this code for the primary surgical intervention when the lesion size necessitates the higher complexity fee compared to Z753 or Z754.
Common Pitfalls
- Billing more than two procedures from the Z753-Z761 group on the same day will result in automatic rejection of the third claim.
- Attempting to bill Z761 for a colonoscopic excision is a common error; these must be billed using the specific colonoscopic codes found on page S19.
- Claiming a standard hospital admission assessment alongside Z761 is prohibited unless it qualifies specifically as the major pre-operative assessment for the procedure.
Billing Tips
- Always append add-on code E688 if laser debulking is performed in conjunction with the electrocoagulation to maximize the claim value.
- Ensure the operative report explicitly documents the base size of the lesion as greater than 5 cm to justify the use of Z761 over smaller-base codes.
