This is an add-on procedure for advancing the colonoscope into the terminal ileum. It is payable in addition to the following primary colonoscopy procedure codes: Z491, Z492, Z493, Z494, Z495, Z496, Z497, Z498, Z499, or Z555.

When to Use

  • Use E705 when you successfully intubate the terminal ileum during a diagnostic or screening colonoscopy (Z491-Z499) to evaluate for Crohn's disease or other ileal pathology.
  • Apply this code when performing a surveillance colonoscopy (Z555) for inflammatory bowel disease where visualization of the terminal ileum is required for mucosal assessment.

Common Pitfalls

  • Billing E705 without a valid primary colonoscopy code (Z491-Z499 or Z555) will result in an automatic rejection.
  • Claiming E705 when the scope only reaches the cecum or ileocecal valve without documented entry into the terminal ileum is considered an audit risk for upcoding.
  • Attempting to bill E705 alongside E740 or E741 is redundant, as those codes already include the procedural work of the colonoscopy.

Billing Tips

  • Ensure your procedure note explicitly states 'terminal ileum intubated' to support the claim in the event of a post-payment audit.
  • Always submit E705 on the same claim as the primary colonoscopy code to ensure the system recognizes the add-on relationship.
