E747 – Endoscopy of ileostomy or colostomy to cecum

An add-on to an endoscopy of an ileostomy/colostomy or a colonoscopy, payable when the procedure is extended to the cecum. This must be billed in conjunction with an appropriate primary procedure.
When to Use
- Use E747 when performing a diagnostic endoscopy of a stoma (Z512) and the scope is advanced through the stoma to reach the cecum.
- Use E747 as an add-on to a standard colonoscopy (Z491-Z499, Z555) when the procedure involves an ileostomy or colostomy and the scope reaches the cecum via the stoma.
Common Pitfalls
- Billing E747 without an eligible primary procedure code (Z512 or Z491-Z555) will result in an automatic rejection.
- Attempting to bill E747 for a standard colonoscopy that does not involve a stoma is an audit risk, as it is specifically restricted to ileostomy or colostomy procedures.
