E890Complex endonasal endoscopic resection from cavernous sinuses

OHIP Surgical Assists Code — NEUROLOGICAL SURGICAL PROCEDURES · Schedule of Benefits

An add-on fee for a complex endonasal endoscopic resection that extends from the cavernous sinuses. This service is only eligible for payment to a physician who has completed a fellowship in skull base surgery or has equivalent experience. It must be rendered with either N114 or N116, and also requires that either E887 or E888 be billed concurrently.

When to Use

  • Use E890 when performing an endoscopic endonasal resection of a non-pituitary skull base lesion, such as a meningioma or chordoma, that specifically involves the cavernous sinus.
  • Apply this code only when the primary procedure is billed as N114 or N116, and the surgical approach requires the concurrent billing of E887 or E888.

Common Pitfalls

  • Failure to bill E887 or E888 concurrently will result in an automatic rejection of the E890 claim.
  • Billing E890 without the primary N114 or N116 procedure code is a common error that leads to claim denial.
  • Attempting to claim E890 for pituitary-only lesions is inappropriate, as this code is intended for complex extensions into the cavernous sinus.

Billing Tips

  • Ensure your operative report explicitly details the cavernous sinus involvement to justify the use of this complex add-on code during potential audits.
  • Verify that your fellowship or equivalent skull base surgery credentials are on file with the Ministry to avoid eligibility-based rejections.
