One or more parts of routine urinalysis (any of SG, pH, protein, sugar, haemoglobin, ketones, urobilinogen, bilirubin) without microscopy. A simple office laboratory procedure that may be delegated. Not insured when rendered for the monitoring of adverse effects resulting from a calorie restricted weight loss program.
If delegated, the medical record must be dated, identify the non-physician performing the service, and contain a brief note on the procedure performed by the non-physician.
