G454 – Stroboscopy
OHIP Diagnostic & Therapeutic Procedures Code — DIAGNOSTIC AND THERAPEUTIC PROCEDURES · Schedule of Benefits
G454 is a diagnostic procedure. As per the general payment rules, services without a specified maximum are limited to one service per patient per day. This service is eligible for special visit premiums when the criteria are met.
When to Use
- Use G454 when performing a stroboscopic examination of the vocal cords to evaluate mucosal wave patterns in patients with suspected dysphonia or vocal fold lesions.
- Select G454 specifically for the stroboscopic component of a laryngeal evaluation, distinct from the standard flexible or rigid laryngoscopy billed under G451.
Common Pitfalls
- Billing G454 in conjunction with G451 is often flagged for audit unless the clinical notes clearly differentiate the stroboscopic analysis from the routine diagnostic laryngoscopy.
- Submitting G454 as a standalone procedure without documenting the specific findings of the mucosal wave analysis will lead to recovery during a Ministry audit.
Billing Tips
- Ensure the stroboscopic equipment is utilized and the resulting mucosal wave assessment is documented to justify the G454 fee over a standard laryngoscopy code.

All insured services must be documented in appropriate records. The Act requires that the record establish that: 1. an insured service was provided; 2. the service for which the account is submitted is the service that was rendered; and 3. the service was medically necessary.
