When to Use

  • Use S589 for a standard unilateral orchidectomy for benign conditions such as torsion, trauma, or chronic pain.
  • Use S589 for simple orchiectomy in cases of androgen deprivation therapy for prostate cancer, as it is distinct from the radical approach required for S598.

Common Pitfalls

  • Billing S589 in conjunction with S596 will result in a rejection, as the insertion of a testicular prosthesis is specifically restricted when performed with this code.
  • Confusing S589 with S598; S598 is reserved for radical orchidectomy involving high ligation of the spermatic cord for malignancy and carries a higher fee.

Billing Tips

  • If a testicular prosthesis is inserted during the procedure, you must claim E834 as an add-on to S589 to receive the additional $50 fee rather than attempting to bill a separate surgical code.
