Represents the application of a cast, as listed under 'COMMON PROCEDURAL DESCRIPTION' in the Schedule on page . This procedure is eligible to be delegated to a physician's employee, such as a nurse or medical assistant, in the physician's office. Specific conditions for delegation must be met as outlined on page of the Schedule.
When to Use
- Use Z873 for the application of a custom-molded cast brace that requires active physician or delegated staff involvement beyond simply fitting a prefabricated device.
- Use this code when the clinical scenario involves the active stabilization of a fracture or joint injury where a bespoke cast brace is constructed to fit the patient's specific anatomy.
Common Pitfalls
- Do not bill Z873 for the application of prefabricated splints, braces, or off-the-shelf casts, as these are explicitly excluded from payment under the Schedule.
- Avoid billing Z873 if the service was performed by staff employed by a hospital or Family Health Team, as the delegation rules strictly limit this to staff employed directly by the physician's private practice.
- Do not bill Z873 in conjunction with Z198 or Z201 for the same encounter, as these codes are mutually exclusive regarding the application of prefabricated versus custom-molded supports.
Billing Tips
- Ensure your clinical notes explicitly describe the construction process of the cast brace to justify the procedure code over a simple splinting code like Z201.
