Z201 – Finger cast application

This service covers the application of a plaster cast to the full arm. As a surgical procedure, it is eligible for age-based premiums for patients under 16 years of age and after-hours premiums for non-elective services. The procedure can be delegated to a qualified employee in the physician's office, subject to the rules outlined in the General Preamble (-).
When to Use
- Use Z201 specifically for the application of a plaster cast to a finger when it is a subsequent application, as it is ineligible if applied during the first 2 weeks of a fracture or dislocation where a procedural fee was already claimed.
- Use this code for definitive plaster immobilization of a finger injury; do not use it for simple metal finger splints or Jones bandages, which are considered part of the initial visit fee.
Common Pitfalls
- Billing Z201 for the application of a prefabricated cast, brace, or splint will result in a rejection, as the code is strictly for plaster cast application.
- Claiming Z201 during the initial 2-week post-fracture or post-dislocation period is a common audit risk, as the application is considered included in the initial procedural fee for the injury.
- Attempting to claim Z201 when the cast is applied at the time of the primary surgery is prohibited, except in the specific case of a cast brace.
Billing Tips
- Ensure the procedure is documented as a plaster application to distinguish it from non-billable prefabricated splinting or simple soft tissue immobilization.
- If the patient is under 16, always append the appropriate age-based premium to the Z201 base fee to maximize the claim value.

Corrective splints must be corrective to qualify for a benefit as such. The corrective splint listings are not applicable to simple immobilization such as with a Jones bandage or metal finger splint following soft tissue injury.
Application of plaster casts or corrective splints are not to be claimed if applied at the time of surgery (except for the application of a cast brace) or applied during the first 2 weeks for a fracture or dislocation when a procedural fee is applicable. The subsequent application of plaster casts may be claimed according to the following Schedule.
Z201, Z198, and Z873 are not eligible for payment for application of a prefabricated cast, brace or splint.
