E984Glaucoma filtering procedure following previous procedure

This is an add-on fee code billed in addition to E132 (Glaucoma filtering procedures ab externo), E114 (Plate and tube glaucoma drainage procedure), or E115 (Minimally invasive glaucoma surgery or minimally invasive bleb surgery) to account for the increased complexity when the procedure is performed following a previous glaucoma filtering procedure on the same eye. This code must be claimed with E132, E114, or E115.

When to Use

  • Use E984 when performing a repeat trabeculectomy (E132) on an eye that has previously undergone a filtering procedure.
  • Apply this code when placing a secondary glaucoma drainage device (E114) in an eye that already has a history of prior filtering surgery.
  • Use this code when performing a revision or repeat MIGS procedure (E115) in an eye where a previous filtering procedure has failed.

Common Pitfalls

  • Claiming E984 as a standalone code; it is strictly an add-on and will be rejected unless billed with E132, E114, or E115.
  • Applying E984 to a primary glaucoma procedure where no prior filtering surgery has occurred, which constitutes an invalid claim.
  • Failing to ensure the primary procedure code (E132, E114, or E115) is present on the same claim, as the Ministry requires the link for the complexity premium.

  • Ensure your operative report clearly documents the history of the previous filtering procedure to justify the complexity premium if audited.
