Debridement of four or more wound(s) and/or ulcer(s) extending into subcutaneous tissue. The service requires a minimum of 10 minutes of debridement and must be rendered personally by the physician. All wound and ulcer debridement services include the application of any necessary dressing if rendered.
When to Use
- Use Z083 when you personally perform debridement on four or more distinct wounds or ulcers that specifically extend into the subcutaneous tissue layer.
- Choose Z083 when the total time spent debriding these four or more lesions meets or exceeds the 10-minute threshold, ensuring each lesion is documented individually.
Common Pitfalls
- Billing Z083 for superficial wounds that do not reach the subcutaneous tissue, which will result in a rejection or audit recovery.
- Failing to document the specific depth and location of each of the four or more wounds, which is a mandatory requirement for this code.
- Billing Z083 in addition to a primary surgical procedure on the same site, as debridement is considered part of the global surgical fee unless complications arise.
Billing Tips
- Always append the E542 premium if the procedure is performed in a clinic or office setting outside of a hospital to increase the total reimbursement.
- Ensure your clinical notes explicitly state the time spent debriding, as the 10-minute minimum is a strict requirement for the validity of the Z083 claim.

Documentation supporting the debridement of each separate lesion for which a claim is made is found in the medical record.
Wound dressing and wound and debridement services are not payable in addition to any surgical procedure unless complications require such care in excess of the usual post-operative care.
