Debridement of two wounds and/or ulcers that extend into the subcutaneous tissue. This service includes the application of any necessary dressings if rendered. Payment requires that the physician personally renders the service for a minimum of 10 minutes, and that the time and details for each lesion are documented in the patient's medical record. When to Use
- Use Z081 when you personally perform debridement on exactly two distinct wounds or ulcers that both extend into the subcutaneous tissue.
- Choose Z081 over Z080 when the depth of the two lesions clearly involves the subcutaneous layer, as Z080 is reserved for superficial lesions.
Common Pitfalls
- Billing Z081 alongside laceration repairs (e.g., Z154, Z175) will result in automatic rejection as these are restricted combinations.
- Failure to document the specific time spent (minimum 10 minutes) or the depth of each individual lesion is a frequent cause of audit recovery.
- Attempting to bill Z081 in addition to a primary surgical procedure on the same site is disallowed unless you document complications requiring care beyond standard post-operative management.

- Ensure your documentation explicitly states the depth of each lesion reaches the subcutaneous tissue to justify the code selection over Z080.

Documentation supporting the debridement of each separate lesion for which a claim is made is found in the medical record.
Wound dressing and wound and debridement services are not payable in addition to any surgical procedure unless complications require such care in excess of the usual post-operative care.
