Debridement of three wounds and/or ulcers extending into subcutaneous tissue. The service includes the application of any necessary dressing if rendered. This procedure is only eligible for payment if the physician performs a minimum of 10 minutes of debridement and the service is rendered personally by the physician.
When to Use
- Use Z082 when you personally perform at least 10 minutes of active debridement on exactly three distinct wounds or ulcers that penetrate into the subcutaneous tissue.
- Select Z082 over Z081 when the clinical documentation confirms the debridement of a third lesion, provided the total time spent meets the minimum 10-minute threshold.
Common Pitfalls
- Claiming Z082 alongside laceration repairs (e.g., Z154, Z175-Z179) will trigger an automatic rejection as these are mutually exclusive services.
- Failing to explicitly document the 10-minute minimum duration or the specific depth of each of the three wounds into the subcutaneous tissue is a frequent cause of clawbacks during audits.
- Billing Z082 in addition to a primary surgical procedure is prohibited unless you document specific complications that necessitate care beyond standard post-operative management.
Billing Tips
- Always append the E542 premium if the procedure is performed in your office or a non-hospital setting to capture the additional $11.55 fee.
- Ensure your chart notes clearly delineate the debridement process for each of the three sites to satisfy the requirement that the service was rendered personally by the physician.
